Today, an in-depth evaluation of the Hospital to Home project has been published.
The project was designed to identify and improve care pathways from hospital to home in the Tayside region of Scotland. Iriss worked with health and social care practitioners and people with experience of the pathway to identify issues to be addressed. This resulted in three broad recommended interventions from the project Working Group that could be adapted and embedded locally with local partners from the case study areas of South Angus and Dundee.
